Liability Waiver and Photo Release

I hereby acknowledge that I understand there may be some risks involved in the making of stained glass art as part of the class I have contracted with Theresa Cashman, owner of TLC Stained Glass (including, but not limited to cuts, burns, and lead exposure). I hereby waive any and all legal or monetary claims for damage or injury to myself or member of my family or my possessions, and assume all legal and financial responsibility myself for such. I shall not now nor in the future make any claim against Theresa Cashman or TLC Stained glass, or any of her family, partners, or helpers for any injury to persons or damage or destruction of possessions or property belonging to myself or others beyond the value of the services performed and agreed in advance. I also grant permission to Theresa Cashman and her colleagues to take and share photos on social media and other platforms, as well as acknowledge that TLC Stained Glass owns such photos and may use them for marketing/publicity purposes. TLC Stained Glass agrees not to use client’s name in connection with public release of photos unless Client gives permission. Client agrees to periodic emails from TLC Stained Glass.
